Application Documents

The following supplemental application documents are to be submitted to the Admissions Office.

Transcripts & School Records

  1. The Release of Records form will be accessible when you start an online application. Complete the top portion of the Release of Records request form and submit the form to the applicant's current school.
  2. The applicant's current school should then forward the applicant's transcripts for the current school year plus 2 previous years of enrollment, standardized test scores, discipline reports, and medical records to St. Dominic Savio.  
  3. Please note: If your child has attended more than one school in the last three years, a Request for Release of Records form must be sent to each school. 

Referral Forms (Recommendations)

  1. St. Dominic Savio requires recommendations from the following individuals for the current school year: English or Social Science Teacher, Math Teacher, and School Principal or Counselor
  2. These Referral Forms (Recommendations) will be sent directly to the individuals you indicate by entering their names and email addresses through the online application then submitting the online application.

Important Note Regarding Referral Forms - These forms WILL NOT be emailed to the respective individuals until the Online Application has been completed and submitted. Please complete and submit your online application as soon as possible to ensure that teachers have adequate time to complete and submit the completed referral forms to the Admissions Office.

Student Essays

All student applicants are required to submit two handwritten essays using the essay prompts provided in the online application. Applicant essays can be uploaded to the online application, emailed, faxed, or mailed.

Immunization and Health Records

Every student enrolled in a Catholic school in the Diocese of Austin must be immunized against vaccine-preventable diseases according to the schedule for schools adopted by the Texas Department of Health. The only acceptable exemption is for medical reasons, which must be documented by a Texas licensed physician. A copy of student immunization records, vision screening, hearing screening and scoliosis screening (males 13 years of age only) must be submitted with the application. International Applicants

The supplemental documents listed above are also required of international applicants and must be translated. All supplemental documents must be translated and submitted to the Admissions Office.

International Applicants Additional Required Documents

  • Copy of applicant’s current passport
  • Proof of English proficiency - International applicants must submit scores from either the TOEFL, IELTS or equivalent.
